The Michigan Department of Health and Human Services has issued a new form to be used to guide care in the event of a medical emergency starting today.

MI-POST is an optional medical order that is part of the advance care planning process and includes choices about cardiopulmonary resuscitation, critical care and other desired interventions. It is intended for individuals with serious advanced illness or frailty.

“The MI-POST form allows families to guide care for individuals who are not able to direct their own care during a medical crisis,” said Dr. Natasha Bagdasarian, MDHHS chief medical executive, in a news release. “It’s a critical component of advance care planning.”
